How do I qualify for Sliding Scale appointments?

I believe that access to safe, compassionate care should be a right, not a privilege. One of my core values as a healthcare practitioner is increasing access to quality care and contributing to a mutual aid and community care model. I extend a sliding scale pay model to those who otherwise may not be able to access my care, including those in the Trans community, QTPOC, uninsured or under-insured individuals, and those who may be facing financial hardship at this time.

There’s no need to “qualify,” just tell me at the time of booking that you’d like to pay sliding scale and tell me how much you can afford. If you find that the sliding scale is more than you can afford at this time, contact me.

Do you take insurance?

No. I provide occupational therapy on a “fee for service” basis as an out-of-network provider. If you have PPO insurance I may offer courtesy billing to your insurance company for out-of-network coverage. This means that clients pay the appointment rate at the time of service and the practice will submit to the clients’ insurance on their behalf so they may get reimbursed by their insurance company. Prior to your first scheduled appointment, call your insurance company to completely understand your out of network occupational therapy benefits.

When you call your insurance company, they will ask you for the name of the occupational therapist you plan to see.  At the time of service and payment, we will submit a “superbill” statement to your insurance company for their consideration of reimbursement to you. I will be happy to provide chart notes or other documentation at your–or your insurance company’s–request. The amount of reimbursement you receive will vary according to the terms of your insurance policy.  Some companies may reimburse you at 80%, some at 60%, some at 40%, and some may not reimburse you at all.  I cannot make guarantees or estimates regarding what reimbursement your plan allows.

Do I need a doctor’s referral?

In the state of California, you are able to access occupational therapy without a doctor’s referral for your first 12 visits or 45 days of treatment, whichever comes first. After that, we’ll need a doctor signing off on your treatment plan.

Good Faith Estimate

Under Section 2799B-6 of the Public Health Service Act, you have the right to receive a “Good Faith Estimate” of expected charges for items and services in regards to your medical care if you don’t have insurance plan or are not seeking to file a claim with their plan or coverage. This law applies to any non-emergency healthcare services, including occupational therapy.

You can ask your health care provider for a Good Faith Estimate before you schedule a service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
